New Patient Visit
 
 

Please arrive 30 minutes early for your first office visit, to provide time to complete necessary paperwork.  Alternatively, prior to your visit you may print a New Patient Welcome Packet and complete the forms in the privacy of your home.  In that case you need only arrive at the office 10 minutes prior to your visit to allow our staff to review your forms and complete any missing information.

Whether you complete the forms in the office or at home, please bring your insurance cards, a list of all medications, a recent EKG if available, any recent lab studies, and any other information that might help your physician get to know your health history and provide care for you.

Referrals

If you are an HMO patient, please be aware that your health plan may require you to have an insurance referral which may be obtained directly from them or from your primary care physician.  This may be in addition to your primary care physician's order for the services of a cardiologist. 

It is your responsibility to know if a referral is required and to obtain it prior to your visit.  The referral must be presented at our office prior to seeing one of our physicians.  This requirement is established by your health plan.  We must follow this policy in order to ensure that you receive your health plan benefits.
