If you are a returning patient who has not been seen in the office within the past three years, you will need to complete a New Patient Welcome Packet and follow the instructions for a New Patient Visit. If you have been seen within the past three years and you are returning for a follow up visit, please remember that we will need to ensure that we have the most up-to-date clinical and demographic information for you. Please complete the Established Patient Packet and follow the directions for an Established Patient. It is especially important for you to complete the medication list in the packet, including prescription drugs, as well as any over the counter medications or food supplements that you may be taking. Referrals If you are an HMO patient, please be aware that your health plan may require you to have an insurance referral which may be obtained directly from them or from your primary care physician. This is in addition to your primary care physician's order for the services of a cardiologist. It is your responsibility to know if a referral is required and to obtain it prior to your visit. The referral must be presented at our office prior to seeing one of our physicians. This requirement is established by your health plan. We must follow this policy in order to ensure that you receive all of your health plan benefits. If you received a referral for a previous visit, please talk to our office staff to confirm that your referral has not expired and that you have not exceeded the number of visits allowed on the old referral. If so, you will need to contact your primary care physician or your insurnace plan to obtain a new referral prior to being seen by your cardiologist. |
