Office Appointments The availability of each physician at each office location varies but we will attempt to schedule according to your choice. We know your time is valuable and we make every effort to be on time for appointments. However, there may be patient emergencies in the office or at the hospital which cause schedule delays. We will keep you informed of these. If you are unable to keep a scheduled appointment, please give us at least 24 hours notice so that we may accommodate other patients who need to be seen.
Please arrive 15 minutes early for your first office visit to provide time to complete necessary forms. Please bring your health insurance information, a list of all medications, a recent ECG is available, any recent laboratory studies, and any other information that might be helpful in diagnosing or treating your problem. And, if you are an HMO patient, please bring your referral from your primary care physician.
Your insurance may require you to have a referral form from your primary care physician in order to see our physicians. If a referral is required, it is your responsibility to obtain it prior to your visit. The referral must be presented at our office prior to seeing one of our physicians. This requirement is established by your health plan. We must follow this policy in order to ensure that you receive your health plan benefits.
If you have an acute or sudden problem, please call our office staff and let them know. Our Triage Nurses will help direct you to the most appropriate care. If an office visit is appropriate, we will make every effort to see you that day. Please call before coming to the office.
